How it Works: Linking You to Success

Our Case Management team serves as the central hub for you and your family. Once you reach out, we will evaluate your unique situation to determine which of our specialized programs or community resources are the best fit.

Our Case Managers Coordinate Access To:

  • Counseling & Mental Health Support: Connecting you with therapists for emotional wellness.
  • Mentoring Programs: Linking youth with positive role models for growth.
  • Community Resources: Tapping into school, township, and local agency support.

What We Do For You

  • Tailored Care Plans: We build a roadmap based on your goals and strengths.
  • Resource Navigation: We identify the right providers and explain their specific intake processes. Whether we send the referral for you or walk you through how to contact an agency directly, we stay by your side through the paperwork.
  • Proactive Follow-Up: We don’t just give you a phone number and disappear. We follow up with you and your providers to ensure connections are made.
  • Continuous Monitoring: We stay involved to ensure the care you receive is working for you. 
  • Direct Access: Families can refer themselves directly to our services, removing unnecessary barriers to getting help.

  • Who Can Participate: Any youth or family seeking support.
  • Cost: Case management services at The Bridge are provided at no cost to our families.

Coordinated Care Team

The Coordinated Care Team (CCT) was created in partnership with The Schaumburg Township Mental Health Board, to address the needs of youth struggling with complex mental health and substance use in the community. This team’s implementation and collaboration follows a Multi-Tiered System of Supports (MTSS) Model to assess, refer, and respond to high stress needs youth and families are facing. Serving youth and families within Schaumburg Township, youth are referred to a Community Care Coordinator. Coordinators support families throughout enrollment, focused on meeting needs, providing support, and tracking progress.

Participants that Qualify

High-Risk Youth residing in Schaumburg Township are referred to the Coordinated Care Team. The program supports both the youth and their family to promote stability and long-term success.
